Write in your own words. Use clear, factual language. Stick to one topic per statement - if you need to address multiple unrelated issues, file a separate 21-4138 for each. The narrative can be as long as you need; attach extra sheets if the form's space runs out.

Common mistakes
- ×Mixing multiple unrelated topics in one statement - file one 21-4138 per topic so VA can route each to the right reviewer.
- ×Editorializing about the VA, the rater, or "the system" - sticks to facts; opinions don't move ratings.
- ×Vague timing ("a long time ago") - give month/year even if approximate.
- ×Restating evidence already in the file - add NEW facts or clarification, don't duplicate.
- ×Forgetting to label attached evidence ("see attached letter from Dr. Smith dated 1/15/2026") so the reviewer can cross-reference.
