Section II - Claim Details

For each claim: date of service, foreign provider name and address, diagnosis (what was treated), the service-connected condition this relates to, and the amount paid. Attach itemized bills.
e.g., 03/15/2026; Dr. Carlos Lopez, Madrid Spain; Diagnosis: Lumbar herniation (L4-L5 disc disease, SC 20%); Total charged: 350 EUR, paid in full.
Common mistakes
- ×Linking care to a non-service-connected condition - FMP only reimburses SC-related care.
- ×Not converting to USD on the form - VA will apply the exchange rate; just report the foreign currency amount.
- ×Missing English translation for non-English bills - required.
