Gather before you start

Section I - Veteran Identification

Common mistakes
- ×Not contacting the Caregiver Support Coordinator first - most changes should be coordinated with the CSC before filing paperwork.
Section II - Nature of Change
Block 6What is changing in the PCAFC enrollment?
Describe what is changing: (1) New primary caregiver replacing current; (2) Adding a secondary caregiver; (3) Removing a caregiver; (4) Veteran's care needs have increased or decreased; (5) Address or contact information update; (6) Responding to a VA reassessment request.
e.g., Primary caregiver changing from Jane Smith (spouse) to Robert Smith (son) effective 06/01/2026 due to divorce.
Common mistakes
- ×Not reporting a caregiver change promptly - stipend payments may go to the wrong person if the caregiver change is not processed.
- ×Removing a caregiver without identifying a replacement - the veteran must have a designated caregiver in the program.
Section III - New Caregiver Information (if applicable)

If changing caregivers: the new caregiver's full identifying information. They will need to complete the same eligibility verification and training as the original caregiver.

Common mistakes
- ×New caregiver not completing required VA training before assuming care - VA training must be completed before the stipend transfers.
Section IV - Signatures
